When you arrive, we will:

• Conduct a detailed assessment of your medical and rehabilitation needs
• Introduce you to your care team and our hospital staff
• Orient you to your room
• Show you around our facility 

• We are here to help you with many daily activities, but our goal is for you to become independent with these activities before you leave.

• Ask for our help to move around. If needed, a walker or wheelchair will be brought to your room.

While you’re in our care, you can expect to participate in therapy sessions. Family may also participate in some therapy sessions to help with your return home.

• Your participation in therapy is essential to progressing towards your goals.
• Therapy is a minimum of three hours per day, five days per week or a total of 15 hours each week.
• Your therapy schedule will be provided daily.
• During your stay, you and your family will receive training to help you feel safe and confident at your time of discharge.

• A rehabilitation doctor will visit you a minimum of three times per week. You may also be seen by one of our internal medicine doctors during your stay.

• Nurses specializing in rehabilitation will care for you 24 hours per day.

• Your social worker/case managerwill meet with you and your family to coordinate your discharge and family training.

• Your rehabilitation care team will meet weekly to review your progress. Following this team conference, your social worker/case managerwill give you an update and continue to work with you to develop your discharge plan.

• Any equipment or follow-up therapy you may need after discharge will be discussed with you prior to your discharge date.

• Discharge time is anticipated at 11 a.m. An alternate time can be arranged with your social worker/case manager if needed.

• You will receive written discharge instructions and prescriptions for your medications.

Patients requiring physician-supervised 24-hour rehabilitation nursing care may be eligible to be admitted to Roper Rehabilitation Hospital. You will be assessed for your condition, as well as any other related conditions, during a pre-admission screening within 48 hours before your planned admission.

To be placed in an inpatient rehabilitation program, the 24-hour intensive services must be reasonable and necessary to treat your condition. Certain diagnoses and symptoms will be considered when evaluating the level of care you may need for your specific condition.

Your functional goals and progress timeframes will be defined after admission evaluations are completed. Once you have achieved your anticipated outcomes or reached the best possible recovery in your inpatient rehab program, you will be discharged according to your ongoing care plan. Other reasons for discharge may include a change in the severity of your medical condition or limited progress toward your expected outcome.

Your condition and the progress you make toward achieving your goals determine how long your stay will be. We carefully monitor your progress throughout your recovery. Our average stay is about 12 days. 

You can expect to participate in physical and occupational therapy, and speech therapy sessions if needed, for a total of at least 3 hours a day, 5 days a week. This focus on therapy and rest can help speed up your recovery.

Family participation can be vital to a successful recovery. They learn how to support you and make your return home as safe as possible. Our rehab team will let you know when we encourage loved ones to take part in therapy sessions. 

We send you home with any needed prescriptions for medications and written instructions regarding self-care. Upon discharge, you can expect to continue your therapy in the home setting or in our outpatient clinic. This helps you continue to progress as you work to regain as much independence as possible. We offer convenient outpatient rehabilitation services throughout the Lowcountry.
