Request medical records

Our hospital medical records request process is fully digital and available 24/7. Patients may request their own medical history, as can parents or guardians for minors and authorized caregivers with Power of Attorney. Our secure online portal guides you step by step while protecting your privacy. You can view your records online or request delivery by secure email or mail.

Getting admitted to the hospital

When you enter the hospital, we’ll greet you at the main desk to offer confidential entry into our facility. Insurance coverage and/or payment options can be discussed in private with a trained member of our team. When your registration is complete, one of our volunteers or staff will escort you and your family to your designated room.

Things to remember for registration

  • Remember to have your insurance card or information readily available.
  • Be familiar with your insurance requirements: copays, deductibles, co-insurance, payment limits and unanticipated additional costs.
  • Bring a photo ID
  • Leave valuables at home

Advanced directives

Advance directives help ensure your healthcare wishes are followed if you can't communicate. They allow you to appoint a trusted decision-maker and outline your preferences for treatment and end-of-life care.
