The Greer Transitions Clinic helps adults who do not have health insurance, have limited insurance coverage, or do not have a regular doctor. Our clinic provides care from board-certified physicians, teaches patients about staying healthy, connects them with community resources, and helps them find a primary care provider when the time is right. While we love kids, our services are exclusively for adults.
The Greer Transitions Clinic is here to help you get started on your healthcare journey. We will take time to learn about your needs and connect you with the care and services that are right for you.
While you may visit us for a short time, our goal is to help you find a primary care provider for your ongoing healthcare needs. We'll be here to support you every step of the way.
We'll help you with your immediate healthcare needs. Once you have a primary care provider, that provider will continue your care and connect you with specialists or other services if necessary.
Note: We don't refill narcotics or other controlled substances.
Taking an active role in your healthcare is key to good health. Please show up when you're scheduled. It's ok if you can't make an appointment, but let us know as early as possible so we can offer that time to another patient. Being actively involved in your healthcare is an important step toward maintaining good health. We encourage you to attend all scheduled appointments so you can receive the care and support you need.
If you are unable to make your appointment, please notify us as soon as possible. Providing advance notice allows us to offer that appointment time to another patient who may be waiting for care.

Someone from the emergency room, case management, or the hospitalist service from Roper St. Francis Healthcare will refer you for your first appointment. After your initial appointment, you will be responsible for scheduling future appointments recommended by our doctor.
We offer a range of care services, so during your appointments, you might see the following:
- A social worker or community health worker
- Medical assistants and office staff
- A doctor, nurse practitioner, or both
If you have insurance, it will be filed with your provider, and you will be responsible for your co-pay at the time of service.
If you are uninsured, there will not be a fee, and our social worker and community health worker will help you enroll in healthcare exchanges and/or apply for Roper St. Francis Charity Care.
Monday - Thursday, 8:30 a.m. - 4:30 p.m., Friday, 8:30 a.m. - 12:30 p.m.
You are strongly encouraged to call 24 - 48 hours before your appointment to reschedule.
Call us. We can help determine if you need emergency care or if you can be seen in our clinic the next day.
We don't have walk-in appointments, but we can often get you in the same-day if you call.
A limited supply of samples is available and may be given to you at the time of your visit if available. Our social workers/community health workers can help you get coupons, sign up for medication assistance, and locate medications for you.
Your care team will work on your plan to determine when you are ready to transition to a primary care doctor.
We are staffed with board-certified medical doctors and nurse practitioners who provide adult primary care services with a focus on hospital recovery, congestive heart failure, high blood pressure, diabetes, and general overall health.
We will provide you with lab and/or test orders. Our team may be able to collect your sample – if not, we will direct you to another location for lab work and/or imaging tests. If you need a test like an MRI or an X-ray, we will help you schedule an appointment at a Roper St. Francis imaging location.
- We are not a medical home. We are here to transition your care to a permanent medical home.
- We are not an emergency room, though we want to help you use our ERs appropriately.
- We are not a pain clinic.
