Qualifications for funding

This is an annual funding opportunity. The application period is between Oct 1 – 31st. Recipients are given priority if they align with our goals based on our most recent Community Health Needs Assessment. For previous recipients, there is now a brief progress report at the end of the application that is required to remain eligible for future funding.

Organizations must meet the following IRS guideline requirements to be eligible for a potential funding opportunity:

  • Must be a 501(c)(3)
  • Must be working to positively impact a prioritized need identified in our Community Health Needs Assessment
  • Funds must be directed to program services rather than fundraising events
  • Request cannot have a primary marketing purpose
  • Must have a health tie-in

Note: If you have previously received funding from us, you must complete the "For Previous Recipients Only" section.

If you experience difficulties submitting the online form, you may download and complete the Community Investment Request Word document and email it to [email protected] for review and consideration.

For further questions or any additional information please reach out to Casey Brunson at [email protected].
