Heart failure is a long-term condition that affects how well your heart pumps blood. It can impact your daily life — causing symptoms like fatigue, shortness of breath and swelling — and often requires careful management after a hospital stay.
Our Heart Failure program is designed to support you during that critical time after discharge, helping you recover safely and avoid complications. For the first month after leaving the hospital, you’ll receive structured follow-up care focused on your recovery, medications and lifestyle changes.
You’ll meet weekly with a nurse practitioner who will guide your care and answer your questions. A board-certified pharmacist will also join you on your first visit to review your medications and ensure everything is working well. And since these appointments are available in person at Roper Hospital and Berkeley Hospital, or via telehealth, it’s easy to stay connected to your care team.
