All information collected with remote monitoring is changed into a secret code (encrypted) and sent securely according to HIPAA regulations.

How long you do remote patient monitoring depends on your health conditions and treatment plan. Most patients participate for 30-90 days, or as recommended by their care team. 

You can enroll in our program with an easy four-step process: 

• Referral: First, obtain a referral from a physician or advanced practice provider.
• Assessment: Next, undergo an assessment to determine your suitability for the program.
• Agreement: Once approved, you'll formally agree to join the program.
• Setup: Finally, you’ll be set up with the necessary equipment and a personalized care plan.

We will give you devices that are set up and ready to go. You don’t need internet access because each device has a connection that sends information to your care team.

If any of your readings raise concern, your care team will contact you quickly to check in and set up any follow-up care you need.

Medicare and most private insurance plans cover remote patient monitoring when it is medically necessary.
