Health Risk Appraisal
Worksite Partners can create personalized health risk appraisals to help organizations better manage their healthcare costs.
To create the appraisals, employees fill-out a personal wellness profile. The profile is then scored and returned to each person in a confidential manner. The profile lets the employee know what areas they need to work on and gives suggestions on how to improve their health.
The management will receive Executive Wellness Reports with aggregate data for health risk targeting. Group or individual counseling is also available regarding the health risk appraisal results.
